Tax Forms
During tax season the Library receives tax booklets and forms from both the Federal government and the State of Massachusetts. The Federal government does not send us any forms or instructions other than Form 1040 and Form 1040-SR
If you need other forms, please use the form below to request that we print those forms for you. Once your request has been received, we will notify you via phone or email, and you can pick the forms up at the Circulation Desk.
